Tompkins County Health Dept. Early Intervention program

A Community Health Nurse in this division works in the Early Intervention program as a Service Coordinator. This program works with children birth to age 3 with developmental delays or disabilities by providing a plan of care for therapy needs while meeting the wholistic needs of the family to help their child reach their maximum potential.  Must have critical thinking skills, good organization skills, basic Microsoft office skills and ability to learn software systems. 

Tompkins County Health Dept Community Health Services Division 

The Community Health Nurse will be part of a dynamic team focused primarily on communicable disease surveillance and management, responsible for investigating 60+ reportable diseases in humans residing in Tompkins County (for example Hepatitis A, Measles, Pertussis, and Tuberculosis) and responding to an outbreak when it occurs. This position will include working at immunization clinics at the Health Department and in the community, as well as education related to the transmission, testing, treatment, and prevention of communicable disease to affected persons and the general public.

This promotional opportunity is limited to current employees of the Tompkins County Health Department. The applicant must currently hold, and have continuously held, at least two years of permanent and/or contingent permanent competitive class status in the title of Community Health Nurse.

This is a professional supervisory nursing position involving responsibility for case management of preventive health care and home care services and /or children with special care needs services, as well as provision of services. The work is performed under the general supervision of the Director of Children with Special Needs. The incumbent assists the Director with administrative program functions in the Children with Special Care Needs Division, assists with management of referrals, intakes and supervision of EI teams.  Must have thorough knowledge of the Early Intervention Program.
